A "Parental Consent / Minor Release Form" is REQUIRED for any individual 17 years of age and under who requests entry into the FOX RIDGE SPEEDWAY pit area.

Due to insurance regulations beyond our control, we are unable to accept any 'non-original' forms for "Parental Consent" or "Minor Release". This means that we can neither fax out copies of this form, nor host them on our web site for you to download or print. Unfortunately, this leaves us with the following options only:

1. You can call us at (608)323-3396 and we can mail you one of these original forms to be filled out.
2. You can contact us via our web site Contact Us page and request that we mail you one of these original forms to be filled out.

Unfortunately, both of these options have a further necessity: Unless the form is signed in front of a valid FOX RIDGE SPEEDWAY representative, the form MUST, according to regulations, be notorized.

Therefore, we suggest that you either sign the form here on your first visit to FOX RIDGE SPEEDWAY or sign the form and then have it notorized by a local notory and then bring or mail it to the Speedway. But, remember, if you are mailing in a notorized form, that form MUST arrive before we can admit the minor.

** THE PERSONAL INFORMATION SUPPLIED BY YOU ON THIS FORM (Social Security Number, E-Mail Addresses, Tax IDs, etc.) IS USED FOR TAX PURPOSES AND HELD COMPLETELY CONFIDENTIAL BY FOX RIDGE SPEEDWAY MANAGEMENT. THE NON-PERSONAL INFORMATION (Sponsors, Chassis, Family and Crew Member Names, Occupation, Etc.) may be used by FOX RIDGE SPEEDWAY for public promotional related items (race recap articles, web site profiles, announcer mentions, RADIO and/or TV programs, etc.).
